Judge Healthcare is currently seeking Service Coordinator Clinicians around Nashville, Knoxville and Memphis, TN!
Job Details: Ongoing Contract (W2, Weekly Pay, Benefits Available)
Location: Local travel around the above locations, TN and working from home (Mileage Reimbursed for local travel!)
Hours: Monday-Friday 8AM-5PM CST
Compensation: Competitive and based on experience
Responsibilities:
• Assist telephonic or face-to-face assessments for the identification, evaluation, coordination and management of member’s needs, including physical health, behavioral health, social services and long term services and supports.
• Assists responsible RN in identifying members for high risk complications and obtain clinical data
• Identify members that would benefit from an alternative level of care or other waiver programs.
• Participate in coordinating care for members with chronic illnesses, co-morbidities, and/or disabilities to ensure cost effective and efficient utilization of health benefits
Qualifications & Requirements:
• Requires LPN or LCSW licensure in the state of TN
• Experience in hospital setting, Home Health setting or case management
• 2 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities
